Members must be 18 years or older.

1. Please complete the following form for Joining CPSA and for Renewing Membership. There is a button on this form to indicate NEW or RENEWAL. Then click the SUBMIT button which will send your information to Cindy Haase, Membership Director.
2. On the next page, please click Add to Cart for each item you wish to purchase. You will have a chance to "View Cart" before your final Check Out. The payment section takes place on the secure PayPal site.  You will be returned to the CPSA Website when your purchase is completed.
3. If you are not the member, please enter your name and contact information in the PURCHASER INFO box. Member information goes in next section.
4. If you are registering more than one person, please make separate transactions.

After you submit this information, you will see the CPSA Shopping List. Clicking any "Add to Cart" button takes you directly to the PayPal Shopping Cart. (You do not have to be a PayPal member to use this option.) From there you can return to the CPSA Shopping List by selecting the "Continue Shopping" button. When you are ready to complete your purchase, click "Proceed to Checkout" on the PayPal Shopping Cart. You will then enter your payment information on the secure PayPal site. You will be returned to the CPSA Website when your purchase is completed.
