$20 nonrefundable fee for each cancelled event. NO refunds after July 17, 2010.

Meal Choices: CPSA offers a vegetarian option for each event that includes a meal. Any other special requirements or food allergies are the responsibility of the individual. Please ask your server if you have questions about the ingredients in a menu item.

Step 1. REGISTER WITH CPSA – Complete the following form to indicate your event and meal choices. Then click the “SUBMIT FORM” button at the bottom to send this information to CPSA.
NOTE: If you are registering more than one person, please indicate the name and meal choice for each person for each event.

Step 2. SELECT EVENTS FOR PURCHASE – On the next screen, you will need to select the events you wish to purchase and indicate the number of meals for each event. This may seem repetitive, but this step is necessary to add the events to your shopping cart.
